Prompt reporting is essential to keeping personnel records accurate and the platform secure. Use the channels below to report problems with the Personnel Management System.

System faults and record errors

For general system faults — pages that will not load, incorrect record data, printing problems with Tags, or failed uploads — contact your unit's records officer first. If they cannot resolve the issue, they will escalate it to the MOD IT Helpdesk on your behalf.

Account and access problems

Accounts are provisioned by an HQ administrator on request from your unit system administrator. If your account is locked, or your role does not permit the action you need to perform, speak with your unit system administrator. Never share credentials or one-time codes.

Suspicious activity and security incidents

Report the following without delay through the MOD IT Security channel via your unit system administrator:

  • Sign-in attempts you did not make, or an unfamiliar device on your account.
  • A request from anyone — inside or outside the Ministry — to share your credentials or one-time codes.
  • Unexpected changes to a personnel record you administer.
  • Suspicious e-mails, links, or attachments purporting to come from the PMS.
  • Any lost or stolen device that was used to access the system.

If you believe personal data may have been exposed, treat it as an incident and report immediately — do not wait for confirmation.

What to include in a report

  • Your service number and unit.
  • The time and date the issue occurred.
  • The URL or screen you were on.
  • A short description of what happened and what you expected.
  • Any error message shown (screenshots are helpful).
